Loan Forgiveness & Discharge Applications

  • This forbearance is intended to assist borrowers whose loans are expected to be fully forgiven and who may not maximize their loan forgiveness benefits because they continue to make payments on their loans while performing the five complete consecutive years of teaching service.

• Loan Discharge Application for Total and Permanent Disability
  • A borrower is totally and permanently disabled "if the borrower is unable to work and earn money because of an injury or illness that is expected to continue indefinitely or result in death." Complete this application to apply for a loan discharge.

Federal Loan Applications

• Federal PLUS Loan Master Promissory Note
  • MPN for the federal PLUS loan where the parent is the borrower. Please check with your school's financial aid office before completing this form.
• Federal PLUS Pre-Approval Request
  • Pre-application to see if a parent qualifies for a PLUS loan.
• Federal Stafford Loan Master Promissory Note
  • MPN for the FFELP Stafford loan that must be repaid by the student. Please check with your school's financial aid office before completing this form.
